Possible records to keep include:
- Individualized Education Program (IEP) and other official school service plans
- Evaluation Summary Report (ESR) and audiogram
- Medical records related to the person’s disability
- Report cards and progress reports
- Standardized test scores
- School transcript
- List of school and community activities
- Awards, Certificates of Achievement, Honors
- Social Security documentation/correspondence
- Employment history
- Resume and letters of recommendation
- MN ID card or driver’s license
- Birth certificate
- Passport and Permanent Resident Card (if applicable)
- Insurance cards
- Bank account information
- Summary of Performance
- Employment documents: W-2, W-4, I-9,pay stubs, etc.
- Copies of tax returns
