Records to Keep

In addition to the forms contained in the Minnesota Transition Guide for Teachers of the Deaf/Hard of Hearing and those listed in the Additional Considerations section of this guide, there are certain records that may be kept in a transition portfolio or other file/storage system. Possible records to keep include:

  • Individualized Education Program (IEP) and other official school service plans
  • Evaluation Summary Report (ESR) and audiogram
  • Medical records related to the person’s disability
  • Report cards and progress reports
  • Standardized test scores
  • School transcript
  • List of school and community activities
  • Awards, Certificates of Achievement, Honors
  • Social Security documentation/correspondence
  • Employment history
  • Resume and letters of recommendation
  • MN ID card or driver’s license
  • Birth certificate
  • Passport and Permanent Resident Card (if applicable)
  • Insurance cards
  • Bank account information
  • Summary of Performance
  • Employment documents: W-2, W-4, I-9,pay stubs, etc.
  • Copies of tax returns
